
As part of our standard monitoring checks for the NPLQ Gen 10 qualification, one aspect of the course/ assessment we monitor is candidate pass and fail rates, particularly where ALL candidates fail the same assessment.
What should I do if all candidates fail the same assessment?
Where detailed in the relevant Guidance and Syllabus, the requirement is, a failed candidate report form must be completed by the assessor and submitted to the Compliance Team within five working days.

Why should a failed candidate report be completed?
A failed candidate report should be completed to enable us to analyse data and proactively address issues that may have contributed to the candidate's failure, for example:
- The Trainer incorrectly taught skills during the course
- The Assessor did not assess in line with the current assessment criteria
There are sometimes occasions where all candidates have failed the assessment however there is not a trainer or assessor error, for example:
- Candidate domino effect
- Candidates have all failed different elements of the assessment
In all instances, a failed candidate report form should be submitted even if the scenario differs from those examples above.
What information is required for the failed candidate report?
- ATC/P name and society number
- Course reference number (if known)
- Trainer/Assessor name and society number
- Details of the failed areas
- Copy of assessment paperwork
If candidates fail, can they just be reassessed on the failed skill(s) and not the whole section?
Firstly, the candidate must be retrained in the area(s) they failed. This training must be recorded accordingly.
A candidate can then be put forward for reassessment. They must be reassessed on the full practical or theory section(s), not just the failed skill(s).
